Linkage Case Manager – FT, Case Management  Position available. Complete Mental Health Assessments for new clients or clients recently discharged from a local hospital or state hospital. Evaluate appropriate services and make all necessary referrals, both within and outside the agency. Meet with hospitalized individuals and provide supportive counseling and aid in crisis stabilization. Collaborate with staff from state operated facilities, support clients in recapturing their daily living skills, fulfill all necessary application requirements for BRIDGE program, maintain client care management records, etc. Provide accurate, timely documentation within a maximum of 48 hours after service delivery. Spend portion of week completing intakes. Qualifications: Bachelors Degree in Human Service Field and 1 year clinical experience, including crisis intervention and stabilization. Must have the ability to administer CPR and emergency first aid and have a valid Illinois driver’s license, auto insurance and automobile. Bilingual Spanish preferred. Hours:35 hour workweek; Monday – Friday, 9:00 a.m.–5:00 p.m.

Therapist – PT Position available. Part time mental health counseling for HIV positive individuals. This position will be primarily subcontracted to a health clinic and will include working at two sites, in Elgin and in Aurora. Qualifications: Master’s degree in human service field and minimum two years experience providing individual therapy to adults. LCSW or LCPC licensure required. Must be able to work independently. Hours: approx. 15 hours weekly, with flexibility in scheduled hours.

Intensive CST Case Manager – FT, Community Support Team (CST) Position available. Identify the services or programs that will best meet clients’ needs and incorporate them into treatment plan. Provide rehabilitation services, develop treatment plan/assessments and manage crises for clients. Assist clients in recapturing their daily living skills, obtaining/maintaining entitlements, finding housing, developing natural community supports, etc. A minimum of 65% of work time will be in direct service to clients including case management, medication monitoring, medication training, community support group and/or individual, and individual counseling.  Qualifications: Bachelor’s degree in human services field with minimum 1 year clinical experience. Experience in crisis intervention and stabilization, knowledge or how to coordinate and integrate delivery of services. Ability to administer CPR and emergency first aid. Must have valid Illinois drivers license and automobile. Hours: Tuesday - Friday 3:00 p.m. – 10:00 p.m., Saturday 6:00 p.m. – 10:00 p.m. plus on call responsibilities Thursday – Saturday and alternating Wednesdays; 32 hour workweek + on-call.

Billing & Data Entry Associate – FT, Business Office  Position available.  Audit all MCO billing and DMH statistics from progress notes and CIS. Check MCO report for rejections and follow through with appropriate action. Process, correct and submit all state billing. Open and close PEP clients in the computer. Verity and enter income and insurance information for client accounts; review client accounts for billing concerns and updates and issue credit adjustments as needed. Perform various maintenance procedures regarding the CIS program. Qualifications:  High School diploma. Must have: ability to key by touch, experience and proficiency in data entry, and proficiency with numbers. Must be self motivated, detail oriented, accurate, organized and able to multi task.  Excellent communication, interpersonal and follow through skills required.  Hours:  Monday – Friday, 9:00 a.m. – 5:00 p.m.; 35 hour workweek.

Staff Nurse – FT & PT - Crisis Residential, multiple positions available.  Set up and distribute medication for clients.  Counsel, coordinate treatment plans, and manage crises for assigned clients. Assess/evaluate and treat minor medical problems for Crisis Residential clients, and assist in scheduling medical appointments and lab tests in the community when necessary.  Qualifications: valid Illinois RN License.  Nursing experience in a mental health setting preferred.  Must have valid Illinois driver’s license. 

Schedules available:

Monday – Friday (nights) 12:00 a.m. – 8:30 a.m. (FT, 40 hour work week)

Saturday - Sunday (nights) 8:30 p.m. – 8:30 a.m. (PT, 24 hour work week)

Saturday - Sunday (days) 8:30 a.m. – 8:30 p.m. (PT, 24 hour work week)
