Bilingual Intensive Case Manager/Assessment Specialist - FT, Case Management Position available. Assist adults with persistent or recurrent mental illness live independently in the community via locating housing, food & transportation resources, applying for entitlements, providing counseling and life skill training and advocating for the client's needs. Complete comprehensive mental health assessments and re-assessments. Develop goal oriented ITP’s that are strength-based and recovery oriented on all assigned clients as required. Provide accurate, quality documentation within a maximum of 48 hours after service delivery. Qualifications: Bachelors Degree in Human Service field and minimum 1year clinical experience. Bilingual Spanish required. Must have a valid Illinois driver's license, auto insurance and automobile. Hours: Monday – Friday, 9:00 – 5:00, 35 hour workweek.
Mental Health Professional – PT Multiple positions available. Provide rehabilitation services, contribute to and develop treatment plans and manage crisis for all residential clients. Handle referrals and discharges for residential services. Assist clients in obtaining/maintaining entitlements. Qualifications: Bachelor’s Degree in Human Service Field or five years’ experience in Mental Health field. Willingness to work regularly scheduled hours, holidays and overtime in emergency situations. Must have the ability to work cooperatively with mentally ill adults, and to administer CPR. Must have valid Illinois driver’s license, auto insurance and automobile. Please specify which schedule is of interest.
SAP: Monday - Friday, 4:00 p.m. - 8 p.m. (20 hours weekly)
CILA: Pool, 8 hour shifts as needed; must be available to work at least 2 shifts monthly
