Option 1
Phone Consultation
1. Call Our Intake Department: Contact us at (847) 695-0484 (prompt two) for a brief phone consultation.
2. Provide Insurance Information: During the call, share your insurance details.
3. Approval and Scheduling: If approved, schedule your appointment or walk in between 12-4 PM, Monday-Thursday, at 1845 Grandstand Place.

Walk In

Walk-In Hours: Visit the reception on the 2nd floor to begin the process. Our walk-in hours are Monday thru Thursday between 12-4 PM at 1845 Grandstand Place, Elgin IL. 60123

No Prior Authorization Required: Simply walk in and complete the intake process from start to finish.

What to Expect
1. Initial Assessment: Complete the new client intake paperwork and speak with a clinician.
2. Personalized Recommendations: Based on your assessment, receive treatment recommendations tailored to your needs.
*Note on Fit and Network: Sometimes, clients may not be a great fit for our services or may not be in our insurance network. We discuss these details with every new client to help them decide whether to proceed with the initial assessment.
